A $150,000 single filer lands in IRMAA tier 2: $405.80/mo Part B + $37.50/mo Part D surcharge, or $2,884.80 a year over the standard premium. IRMAA — the Income-Related Monthly Adjustment Amount — is a surcharge on Parts B and D for higher earners. It runs on a two-year lookback: your 2024 MAGI (AGI + tax-exempt interest) sets your 2026 premiums. The surcharge is cliff-based — $1 over a line moves you a full tier — and per person, so a couple both on Medicare pays each amount twice. Income drop since 2024 (retirement, divorce, death of a spouse)? File Form SSA-44 to appeal on current income.
| 2024 MAGI, single | 2024 MAGI, joint | Part B total/mo | Part D surcharge/mo | Extra/yr vs standard |

|---|---|---|
| Initial enrollment | 7 months: 3 before, the month of, and 3 after your 65th birthday | Miss it and late penalties can follow you for life |
| General enrollment | January 1 – March 31 (coverage starts July 1) | The fallback if you missed your initial window |
| Open enrollment | October 15 – December 7 | Switch Advantage plans or return to Original Medicare |
| MA open enrollment | January 1 – March 31 | Advantage members: switch plans or drop to Original Medicare |

| IRMAA tier | 2024 MAGI vs the 6-tier table | $150k single → $405.80/mo Part B + $37.50/mo Part D. |
| Annual extra vs standard | (tier premium − $202.90 + Part D surcharge) × 12 | $2,884.80/yr at the defaults — the price of that tier. |
| Annual Medicare bill | 12 × (Part B + Part D) + $283 + expected cost-sharing | $4,257.80 at the defaults. |
| Hospital stay | $1,736 + $434 × days 61–90 + $868 × reserve days | 5 days = $1,736; 70 days = $6,076. |
All premium/deductible/IRMAA figures are the official CMS 2026 amounts (released November 14, 2025). MA premiums, Medigap premiums, and standalone Part D premiums vary by plan, age, and ZIP — the chart uses national averages. IRMAA uses 2024 MAGI; the lookups assume standard thresholds (married-filing-separately has its own harsher schedule). The hospital-stay lookup prices Original Medicare cost-sharing; Advantage plans use their own copay schedules up to the $9,250 cap. Not financial or insurance advice.
Bright Side Kit. "2026 Medicare Costs Chart & IRMAA." https://brightoolbox.com/2026-medicare-costs-chart/ Accessed October 23, 2026.The standard premium is $202.90 a month — the first time it has ever exceeded $200 — up $17.90 from $185.00 in 2025. The annual Part B deductible is $283. Higher earners pay IRMAA surcharges on top: see the table above or run Lookup 1 with your 2024 income.
IRMAA starts when your 2024 MAGI exceeds $109,000 single or $218,000 married filing jointly. The first tier raises Part B to $284.10/mo and adds a $14.50/mo Part D surcharge — $1,148 a year over standard. It is cliff-based: $109,001 moves you the full tier. The top tier ($500k+ single) totals $689.90 + $91.00 a month.
The Part A deductible is $1,736 per benefit period — and a benefit period is not a calendar year, so two stays 60+ days apart cost $1,736 twice. Days 1–60 are covered after the deductible; days 61–90 cost $434/day; lifetime reserve days cost $868/day. A 5-day stay is $1,736; a 70-day stay is $6,076. Medigap Plan G covers the deductible; Advantage plans use copays up to the $9,250 cap.
$2,100 a year for covered drugs — once your spending hits it, covered prescriptions cost $0 for the rest of the year. The old coverage gap (“donut hole”) is gone. Standalone Part D plans may charge a deductible up to $615.
Yes. If a life-changing event — retirement, divorce, death of a spouse, loss of income-producing property — cut your income after the 2024 tax year Medicare is using, file Form SSA-44 with Social Security to have IRMAA recalculated on your current income. Roughly 8% of enrollees pay IRMAA, and many do not know the appeal exists.
October 15 – December 7, 2025 for 2026 coverage — the window to switch Advantage plans, join or drop Part D, or return to Original Medicare. A separate Medicare Advantage open enrollment (January 1 – March 31) lets current Advantage members switch plans once.
